C4 is developing a community care campus that brings together nonprofits to improve access to services for individuals and families trying to navigate resources.
C4 is a connector and coordinator.
We are not a clinical provider and do not replace the direct services offered by existing nonprofits.

C4 exists to foster collaboration among nonprofits and create stronger community-based networks of care, support, and resources.

Tina Harris
Tina Harris, DNP, MS
Dr. Harris directs cancer care resources for the underserved community offering programs for qualifying underserved/non-insured cancer patients. Served on the health disparities task force, Advanced Practice Society Hematology Oncology board, national speaker on cancer prevention
and access to healthcare, researcher, publisher, and developed the financial navigation screening program.
